If you are the parent of a child or young adult (aged 13-18) with disabilities, you can complete an application form for them to become a member of Phab Nottingham here.

 

Once submitted, you will receive a call from one of the Phab Nottingham team. After your application has been reviewed, you will be added to our mailing list so you can be kept up-to-date on our upcoming events!
